Eligibility Requirement
All consumers desiring to receive services from One Love Developmental Services must be Medicaid eligible for services. The consumer and/or guardian must apply for services through a local LME or Area Authority or Designee for Case Management Services. One Love Developmental Services must have a contract with the LME within the consumer catchment area to provide services. The consumer or parent will be asked specific questions regarding the client's diagnosis, psychological/psychiatric histories, hospitalization, social history, education, vocation, and medical history.
The Case Manger along with the Interdisciplinary Team will meet with the consumer and the Provider of Choice to discuss the consumer's needs, strengths, and goals to help the consumer with deficits and assist them in becoming as independent as possible. If One Love Developmental Services, Inc. is chosen as a Provider, the guardian or consumer must complete a screening assessment with identifiers to determine whether or not the organization can provide the needed service(s). If One Love decides that services can be provided, then the consumer along with his or her guardian will interview a staff person to work with him. A consumer satisfaction survey will be completed monthly to assess the consumer's satisfaction of service(s).
