Community Based Services (CBS)
Provides a variety of activities in the community that tailors to the individual needs of the consumers and to assist in the development of skills for appropriate goals.
Alternative Family Living
Targets consumers to promote maximum growth and independence and provide avenues for an individual to make positive personal choices.
Supported Living
Provides support needed for an individual in order for him/her to live in the community as independently as possible.
Day Habilitation
Provides direction instruction to the consumer to increase independent living and social skills needed for basic interactions with others in the home and community.
Respite Care
Provides temporary relief of families from the daily care and management of the CAP-MR/DD/MI Client.
MR Personal Care
Include assistance with eating, bathing, dressing, personal hygiene, and activities of daily living.
Individual Apartment Living
Provides assistance to individuals who have a Mental Illness, Developmental Disability, or Substance Abuse who lives in an apartment and requires minimum assistance with day to day care.
Supervised Living
Provides residential services to individuals in a home environment where the primary purpose of these services is he care, habilitation or rehabilitation of individuals who have mental illness, a developmental disability or disabilities, or a substance abuse disorder, and who require supervision when in the residence.
MR/MI
Provides services to adults with mental retardation, or who have been treated as if they had mental retardation, who were admitted to a state psychiatric hospital on or after March 22, 1984, and who are included on the Division of MH/DD/SA Services official list of prospective adults with Mental Retardation who have a documented history of State hospital admission regardless of admission date or have never been admitted to a State Psychiatric Hospital but who have a documented history of behavior determined to be of danger to self or others that results in referrals for impatient psychiatric treatment; and without funding support, have good probability of being admitted to a State Psychiatric hospital, Expenditures for services to clients listed in Subparagraph (a)(2) of this rule are limited by legislation and require specific approval by the Division.
Personal Assistance
Provides aid to a client who has a Mental Illness, Developmental Disability, or Substance Abuse Disorders so that the client can engage in activities and interactions in which the client would otherwise be limited or from which the client would be excluded because of a disability or disabilities.
Residential Level II & III
Provides a structured living environment for children and adolescents who have a primary diagnosis of mental illness or emotional disturbance and who may also have other disabilities and for whom removal from home is essential to facilitate treatment.
Crisis Stabilization
Provides close supervision to the person on an individual basis and assists during periods of time in which the person is presenting episodes of unmanageable and/or inappropriate behaviors, which require specialized staff intervention.  An individual may display extreme, maladaptive behaviors that are not anticipated, are temporary in nature and are beyond the daily behaviors, which are addressed through other supports.
Family Training
Provides training and counseling services for the families of individuals served through the waiver.  Training includes instruction about treatment regimens and use of equipment specified in the Plan of Care, and shall include updates as necessary to safely maintain the individual at home.
In-Home Aide
Include general household activities, such as meal preparation and routine household care, provided by a trained level In-Home Aid, when the individual regularly responsible for these activities is temporarily absent or unable to carry out these activities
Supported Employment
Supported Employment is paid employment for persons with developmental disabilities for whom competitive employment at or above minimum wage is unlikely and who, because of the severity or their disabilities, need intensive ongoing support to perform in a work setting.
